    For New Patients Only:
    Appointment Cancellation Fee
    If you are unable to attend your scheduled appointment, please notify our office at least 24 hours in advance. Failure to do so will result in a cancellation fee of $150 for new patient appointments (in order to reschedule your appointment) and $50 for returning patient appointments.

    PLEASE COMPLETE BELOW IF WE DO NOT HAVE YOUR CURRENT INSURANCE ON FILE.  PLEASE BRING ALL INSURANCE CARDS TO YOUR NEXT APPOINTMENT.  IF YOU HAVE A SECONDARY INSURANCE OR PRESCRIPTION CARDS, PLEASE BRING THOSE CARDS AS WELL TO YOUR APPOINTMENT.
